Monto Local Coordinator / Chronic Complex Care Coordinator, Monto Medical Center
Anita Salisbury (Registered Nurse) In my position as Local Co-ordinator at the CQRDGP I have been involved in many projects that are individually run by the division or in partnership with other organizations i.e. Monto Health Service, Monto Primary/High School, Ridgehaven Retirement Complex and Landcare. I enjoy my role as local co-ordinator due to the scope it offers me in Health Promotion, to assist our GP’s to be in contact with their communities and offers the opportunities to work with the other health care providers. Our General Practitioners and the CQRDGP have given our community the opportunity to engage and respond to the health education needs of our primary schools with some great results. We have projects such as Hand washing, puberty clues, cough and cold prevention, sun safety, mental health (2008) and CPR (with QAS). These projects have also been supported by Education Queensland and Queensland Health. We also have an interagency meeting quarterly between the private and public health care providers, which is an opportunity to plan and interact for our communities benefit.In my position as Monto Chronic Complex Care Co-ordinator I have been given the opportunity to use my links within the community to establish better outcomes for those suffering the selected chronic diseases we are targeting. I work with the General Practitioner to ensure that we are developing a management plan that considers the impact of the disease, education towards medications and treatments and the requirements for team care arrangements to further support the patient in the path to better management and outcomes. This is a twelve-month project and we look forward to the outcomes in July 2009.Overall, with the support of Monto Medical Center, the CQRDGP has given Monto and myself a great amount of scope and support to our communities benefit. Given the fact that I am employed by Monto Health Service, Blue Care and the Monto Medical Center in my role as a Registered Nurse the CQRDGP offers the flexibility to fulfill my career, balance my family, farming enterprise and friends and still contribute to the outcomes for our community and the CQRDGP.

Monto Local Coordinator
Carly Burnham has recently joined the division in the capacity of Local Co-ordinator for Monto (shared position with Anita Salisbury) and Project Liaison (PND Training and Development). Carly has a strong mental health background and is an active member of the Monto community. Carly is passionate about establishing strong holistic networks between mental health providers and the community. She has harnessed her artistic talents and is currently raising awareness of mental health issues, with a Queensland touring photographic exhibition in conjunction with AgForce QLD. Carly lives on and runs a cattle property west of Monto with her husband and 3 children

Clermont Local Coordinator
Nea Hamblin is the Local Area Coordinator for Clermont. Nea works full-time at the Clermont Multipurpose Health Service as the Health Promotion Officer and believes the local area coordinator's role is a vital link between the hospital, the surgery and the community. Nea is married to Ash and has a daughter, Tabitha. Nea can be contacted during business hours on (07) 49834012.

Clermont Local Coordinator
Janelle Clancy is the Local Coordinator for Clermont area. She has spent most of her life in Clermont and is now studying a Diploma of Population Health to broaden her knowledge and research of today’s communities, she also has a background in Life Coaching and assists with Lighten Up Programs. Janelle believes in making open & clear communication a priority between GP’s, the local Hospital & the Community. In a Rural area, this is one of the major keys of having positive impact to support Health Professionals in working here and bringing along their families to live a well-adjusted life.

Moranbah Local Coordinator
Rachael Cox-Randall is an Accredited Practising Dietitian who graduated from The University of Newcastle in 2003. She worked for the Barwon Division of General Practice in Moree NSW from 2004 to mid 2006 before moving to sunny QLD. She has worked in the Central Highlands for 4 years as a Dietitian, both as an employee of the Division and as a private practitioner. She has now taken on a part time role as the Healthy Lifestyles Project Coordinator for Moranbah, Clermont, Middlemount, Tieri and Dysart and is looking forward to working more on a health promotion level with community groups.

Emerald Local Coordinator
Joanna Reid is the Local Coordinator for Emerald. She also works part time for the Division of Library Services, and Academic Learning Services of CQUniversity in Emerald. She has previously worked as the Personal Support Program coordinator for Centacare in Emerald. She is currently undertaking her Honours in Psychology and completing a Diploma of Counseling. Joanna grew up in Emerald and currently lives here with her daughter.
