CQRDGP aims to support General Practice in implementing federal initiatives under Medicare in particular the uptake of Health Assessments/Health Checks and Chronic Disease Management item numbers.

• Uptake of GPMPs and TCAs
• Uptake of Health Assessments/Health Checks
• Aboriginal and Torres Strait Islander specific Assessments
• Practice Incentive Payments and Service Incentive Payments
