Lifestyle Modification Programs (LMP)
The Central Queensland Division of General Practice provides Lifestyle Modification Programs for the community. Current programs running in the area include Reset Your Life and Lighten Up.
About the Reset Your Life Program
Reset Your Life is a Lifestyle Modification Program developed by the experts at Baker IDI Heart and Diabetes Institute designed to target patients at high risk of developing type 2 diabetes.
The program’s focus is on helping participants to make lifestyle changes that will ultimately prevent or delay the onset of type 2 diabetes. The program is subsidised by the government and run locally by accredited facilitators.

What does the program involve?
The ‘Reset Your Life’ program consists of a series of group motivational and educational sessions supporting lifestyle and behavioural changes and adoption of healthy lifestyle choices in participants.
Education is provided on lifestyle choices including diet, physical activity and stress management The program comprises of seven sessions which are held over six months and presented by a dietician, health promotion project officer and social worker. Groups can be run with between five and fifteen participants and begin every two weeks. The aim of the program is to help people to make sustainable lifestyle changes.
How will the program benefit you?

Setting goals to improve your health
After three years post participation in a Lifestyle Modification Program, it has been shown that fifty-eight percent of patients at risk of developing type 2 diabetes have avoided the onset of the disease.
Programs will be held in Biloela and other Towns throughout Central Queensland. A one-off fee of $50 will be required to participate in the Reset Your Life program (health care card or concession card holders are exempt from this fee). Face-to-face and telephone support will be provided to participants throughout the program.
Who is suitable for the Reset Your LIfe program?

People with High risk of diabetes with a score of 12 or greater on the Ausdrisk Assessment Tool

The AUSDRISK Tool is a simple 10-question survey designed to assess a patient’s risk of developing diabetes in the next 5 years. The survey is available to anybody, and can be completed either independently, or with the assistance of your health care professional.

How long will the program go for?
Each session duration varies between 1-2 hours. The program is run over a 6 month period and includes 7 group sessions.
The program will begin with intensive meetings held at fortnightly intervals.

Diabetes is a serious chronic disease that is becoming far more prominent than ever before. Whilst the prevalence of diabetes throughout the community continues to rise, it is important to note that the disease is highly preventable. Even better news is that you don’t have to spend a fortune on preventative medications – simply making sensible changes to your current eating habits and activity levels can have dramatic effects. This is where Lifestyle Modification Programs comes into the picture.
If you are interested in a lifestyle modification progrqam, make an appointment to see your GP and take your completed AUSDRISK Tool along with you. Your GP will check to make sure you have not actually developed diabetes by checking a blood sugar test, and then you can be referred into the program. Or you can simply contact your local Health Promotion Project Officer through the Central Queensland Division of General Practice on 4992-5544.
